With the start of the new school year, OCC needs an updated Enrollment/Consent form for every child & youth involved in church activities. This helps us to ensure your child's safety and receive updated info on medications, insurance, allergies, and emergency contacts.
Please fill out this form and return it to the Church Office or Rev. Rob by September 1st!
